This visa is issued to citizens of Italy traveling to India for medical purposes. A letter from the medical institution in India confirming your medical treatment in India is required. If employed, a letter from your employer is required stating his awareness of your leave.  Proof of sufficient funds to cover up all medical expenses. Holders of this medical visa are not allowed to work while they are in India. An official document from the medical institution in India stating that they can provide the medical treatment you need. You are expected to leave the country at the end of your treatment. Holders of this visa are not allowed to take up permanent residence in the country, but a temporary residence. The applicant has to sign a medical declaration form.  If you are under the age of 18, you must possess the original copy of your birth certificate. If you are traveling alone or with one of their parents, a document of consent should be signed by both parents/sponsors. A letter to the medical institution in India is needed. This letter must state the nature of the illness and why treatment is ill-suited elsewhere. Travel itinerary. Payment of medical treatment. Proof the patient can cope with any additional medical costs that may come up. Proof of correspondence between the doctor in your home country and the one in the Schengen area.
